Person-Directed Care at HCH
At Holland Christian Homes, we are committed to creating communities where residents and tenants are known, valued and supported to live life in a way that feels like their own. Person-Directed Care means getting to know each resident and tenant as a person – their story, preferences, routines, relationships and what brings meaning to their life.

Featured Video & Statement
“What matters to you, matters to us.”
Our goal is to create a community where residents and tenants feel known, valued and supported to live life in a way that feels like their own.
What Person-Directed Care Means at HCH
We are working toward a community and neighborhoods where residents and tenants are active partners in decisions about their lives, their homes and the care and services they receive.

Know You
We take time to learn about your story, preferences, routines, interests and what is important to you.

Listen to You
Your voice matters. We listen to your choices, goals and preferences and involve you in decisions that affect you.

Respect Your Routines
Home should feel like home. Wherever possible, we adapt our routines and practices around you, rather than expecting you to adapt to us.

Support Your Independence
We recognize and support what you can and want to do for yourself while providing assistance when you need it.

Create Meaning and Purpose
We want every person to have opportunities for connection, contribution, growth, enjoyment and purpose.

Build Relationships
Meaningful relationships with family, friends, neighbours, Care Partners and the wider community are important for well-being.

Partner With You
Residents, tenants, families and Care Partners each bring knowledge, experience and ideas to our community. We work together while keeping the resident or tenant at the center.
Rooted in Faith. Centered on the Person
Our approach to Person-Directed Care is deeply connected to the Christian faith and values upon which HCH was founded. We believe that every person has dignity and worth throughout every stage of life. Aging or needing additional support does not change who someone is, what they value or what they can contribute.
Our Christian Faith and Values.

Why We Say “Care Partner”

The words we use help shape our culture. At HCH, we are moving toward using the term Care Partner to describe the people who work throughout our community. The word partner reflects the relationship we want to build. Residents and tenants are not simply recipients of care or services. They bring their own knowledge, experiences, abilities, preferences and choices to the relationship.
Bringing Person-Directed Care to Life: Our Eden Journey
HCH is on a journey toward becoming an Eden Alternative certified community. The Eden Alternative provides a framework to help us put Person-Directed Care into everyday practice across HCH.

Our Journey So Far

Building Eden knowledge across HCH
Care Partners and leaders are learning about the Eden philosophy.

Developing Certified Eden Associates
The organization has made significant investments in our Care Partners. We are currently training our Care Partners as Certified Eden Associates and have nearly 300 Care Partners trained to-date.

Listening to residents and tenants
Resident and tenant voices help us understand what matters and where we can improve.

Looking differently at our practices
We are examining routines, language, environments and ways of working.

Continuing toward Eden certification
Our journey continues as we learn, listen and build Person-Directed Care into everyday life.
Person-Directed Care in Action
Person-Directed Care is more than a philosophy. It should be something residents and tenants experience in everyday life. Across HCH, we are finding new ways to put these ideas into action. Follow along on our social media to see stories, photos and updates about Person-Directed Care in our homes and community.

Person-Directed. Evidence-Informed
Faith Manor and Grace Manor are proud Best Practice Spotlight Organizations (BPSO) through the Registered Nurses’ Association of Ontario.
Through our BPSO work, we use evidence-based Best Practice Guidelines to strengthen care and improve resident outcomes in areas including person- and family-centered care, and end-of-life care.

Residents, Tenants & Families as Partners
Residents and tenants are at the center of all we do. Families, friends and substitute decision makers, when applicable, are important partners. We work together to understand what matters to each person and to create a community where everyone can thrive.
